Did you know that you have the right to choose who provides your care? It’s called Patient Choice and it’s a key part of the NHS system. Patient Choice gives you and your family the power to make decisions about your care provider, where you receive treatment and how your recovery is managed.
At CRS, we want to empower you to make informed decisions about your care. Here’s everything you need to know about Patient Choice and how CRS can support you.
Patient Choice is your right to decide:
You can choose your care provider after a hospital stay or illness. This includes selecting organisations like CRS for your reablement and rehabilitation services.
You can discuss your preferences with your healthcare team to create a care plan tailored to your recovery goals.
Whether it’s a hospital, a specialist clinic, or care at home, you can decide what works best for you and your family.
This right is part of the NHS Constitution, ensuring that every patient receives care that respects their individual needs and circumstances.
Choosing your care provider means you’re in control of your recovery.
It allows you to:
Access specialised care tailored to your specific condition.
Work with trusted providers who deliver compassionate, high-quality care.
Receive treatment in an environment where you feel comfortable, such as your own home.
Empowering Loved Ones to Support Your Recovery At CRS, we know that recovery is a team effort. That’s why we offer free access to our Recovery Assistant Training Module for the loved ones of anyone using a CRS reablement package. This training equips your family with the skills and confidence to actively support your recovery at home. Topics include:
Learn correct techniques to assist with mobility and transfers
Minimise risks and help keep your environment safe.
Identify signs of complications or setbacks.
Reinforce therapy between sessions to improve strength and independence.
Understand the mental health aspects of recovery and how to create a positive environment.
The training is simple to follow and available online or in print, allowing your loved ones to learn at their own pace. With CRS, your family becomes part of your recovery team, ensuring you receive continuity of care and support every step of the way.
When you choose CRS, you’re choosing a provider dedicated to your recovery and independence. Here’s what
we offer:
We work with you and your family to design a personalised care plan that meets your needs and preferences.
Our team of trained Recovery Assistants goes beyond traditional care, offering advanced reablement services that include physiotherapy, mobility training and daily living support.
We help you avoid delays in care by starting your reablement package as soon as you’re discharged from the hospital.
Our services are guided by senior therapists to ensure your recovery is effective, evidence-based and aligned with NHS guidelines.
We’ve made getting support simple and stress-free:
Let them know you’d like to choose CRS as your care provider. You have the right to request CRS if we’re available in your area.
Our team can guide you through the process and coordinate with your healthcare providers.
With CRS, you’ll receive the compassionate, personalised care you deserve.
At Care & Rehablement Solutions, we believe the best care is personal, flexible, and centred around you. That’s why we’re proud to support individuals using Personal Health Budgets PHBs.
A Personal Health Budget is NHS funding that gives you more control over how your health and care needs are met. Instead of traditional care arranged entirely by the NHS, a PHB lets you choose the support that works best for your life, your goals, and your recovery.
If you’re eligible, you can use your PHB to pay for things like:
Support workers or personal assistants
Specialist therapies (e.g. physiotherapy, occupational therapy)
Home-based reablement care
Equipment, technology, or adaptations
Alternative ways to stay well and independent
PHBs are often available for:
People receiving NHS Continuing Healthcare (CHC)
Children and young people with complex health needs
People with long-term conditions or disabilities
Anyone who would benefit from a more individualised care approach
If you’re not sure if you’re eligible, we can help you explore your options.
At CRS, we understand the courage it takes to recover. That’s why our team walks alongside you every step of the way — with compassion, skill, and a commitment to your progress.
