Empowering independence through tailored recovery and ongoing support.
Welcome to Care & Reablement Solutions (CRS), where we’re redefining post hospital and ongoing care through truly person centred support.
At CRS, we don’t simply provide carers we provide Recovery Assistants. Our Recovery Assistants are specially trained in therapeutic support techniques and work within structured recovery plans designed to help individuals regain and maintain independence.
Our approach is physiotherapist led, meaning our recovery programmes are designed, overseen and regularly reviewed by qualified physiotherapists. Recovery Assistants are trained and supported by these professionals to ensure that each person receives safe, effective and goal focused support.
We understand that day to day assistance is important, but we believe real care goes further. Our focus is on empowering individuals to do more for themselves, at their own pace, with the right guidance, encouragement and rehabilitation focused support.
CRS supports you at every stage of the journey from initial recovery through to ongoing independence.
We also believe that recovery works best when everyone is involved. That’s why we offer free training for family members, helping loved ones develop the knowledge and confidence to safely support the recovery journey and continue progress between visits.
Our structured recovery packages are designed to reduce reliance on long term care wherever possible, helping people regain skills, confidence and control over their daily lives.

Care & Reablement Solutions CRS is a nationally recognised provider of specialist home-based reablement. We support individuals recovering from illness, injury, surgery, or hospital discharge, empowering them to regain independence, confidence, and control all without leaving the familiarity of home.
We are regulated by the Care Quality Commission CQC in England and ensuring the highest standards of safety, accountability, and professionalism.
As the NHS and social care systems move toward more integrated working through local Integrated Care Systems ICSs, we at CARE & REABLEMENT SOLUTIONS are proud to align our services with these changes. Our goal is to support the NHS, local authorities and families by offering flexible reablement packages that ease discharge pressures, improve recovery outcomes and provide choice through personal health budgets or direct payments.
Our Reablement service helps individuals regain their independence and improve their quality of life after illness, injury, or surgery.
Scheduled visits providing personal care, exercises, mobility support, and recovery-focused routines.
Targeted short-term support to help people regain strength, skills, and confidence following hospital discharge, injury, or illness up to 6 weeks.
Short-term cover for family carers, with a recovery-based approach that keeps progress moving even when the main carer is away.
Those at the end of life would not receive Reablement. CRS will support, empower, and ensure the person's final wishes are delivered with passion, commitment, and dignity.
Reablement is a short-term, intensive form of care designed to help individuals recover skills and rebuild their ability to manage daily life following a health setback. Unlike traditional care, reablement focuses on progress, not dependency helping people regain confidence and independence rather than creating long-term reliance.
At Care & Reablement Solutions CRS, reablement is delivered through a unique combination of:
Physiotherapist led intervention
Recovery Assistant–led daily reablement sessions under clinical guidance
Our goal? To help every person we support achieve meaningful recovery outcomes that last.
We offer far more than standard care we provide a structured, recovery-focused service led by trained professionals who understand what it takes to get better and stay well.
Reablement is typically offered to adults recovering from illness, injury, surgery or hospital admission who need short-term support to regain independence at home. Through the NHS or local authority, it is often provided free for up to six weeks, based on an assessment of need.
However, people also have the right to choose how their reablement is delivered, including using direct payments or a personal health budget to select a provider like CRS. Our packages are designed to work alongside NHS and local authority services, offering additional flexibility, extended support, and improved outcomes.

At Care & Reablement Solutions, we hold the deepest respect for the incredible work carried out every day by the NHS and our colleagues across the health and social care sectors. These professionals are the backbone of our care system, delivering vital, compassionate support to individuals and families across the UK.
Our role is to work alongside the NHS and social care services not as a replacement, but as a supportive partner. We aim to complement their efforts by delivering high-quality, personalised after-hospital care that promotes faster recovery, supports independence, and reduces unnecessary hospital readmissions. By helping individuals transition safely from hospital to home or community-based settings, we contribute to easing pressure on services, including reducing overcrowding and waiting times.
We also recognise that recovery is not just physical it’s social, emotional, and practical. That’s why our approach includes supporting wellbeing, reconnecting people with their communities, and addressing the wider needs that impact long-term recovery and quality of life.
Together with our health and social care partners, we strive to ensure that every person receives the right support, at the right time, in the right place.
At CRS, we believe recovery works best when families feel supported too.
We offer free training and upskilling for family members and unpaid carers, including:
